This short questionnaire is to see if there is any reason for a participant to check in with their healthcare professional before starting a walking program. A participant does not need to pass in this form to you. Participants who are unable to download this form, can go to their local library and they can print off a copy of the form.

: Outlines the responsibility of both the participants and leaders for safety and enjoyment on the walks

: Provides important information on a participant to Hike NS for evaluation purposes and ongoing support to Walk Leaders/Hosts
