These resources are for trained NS Walks Walk Leaders/Hosts:

Get active Questionnaire and the Get Active Questionnaire Reference Document: For walk participants to complete before joining a walking group. This short questionnaire is to see if there is any reason for a participant to check in with their healthcare professional before starting a walking program. A participant does not need to pass in this form to you. Participants who are unable to download this form, can go to their local library and they can print off a copy of the form.

Participant agreement and waiver form: Outlines the responsibility of both the participants and leaders for safety and enjoyment on the walks

Welcome to your first walk form: Provides important information on a participant to Hike NS for evaluation purposes and ongoing support to Walk Leaders/Hosts

Risk assessment form: This form helps Walk Leaders/Hosts assess a walking route before taking a walking group on the route

Walk attendance form: Walk Leaders/Hosts complete this form for every walk
