: For walk participants to complete before joining a walking group. This short questionnaire is to see if there is any reason for a participant to check in with their healthcare professional before starting a walking program. A participant does not need to pass in this form to you. Participants who are unable to download this form, can go to their local library and they can print off a copy of the form.
Participant agreement and waiver form
: Outlines the responsibility of both the participants and leaders for safety and enjoyment on the walks
Welcome to your first walk form
: Provides important information on a participant to Hike NS for evaluation purposes and ongoing support to Walk Leaders/Hosts
Risk assessment form
: This form helps Walk Leaders/Hosts assess a walking route before taking a walking group on the route
Walk attendance form
: Walk Leaders/Hosts complete this form for every walk

COVID-19 Walk/Hike Guidelines
: Recommendations for walking/hiking group best practices during the pandemic
Checking for proof of vaccination
: To comply with new provincial Covid 19 regulations introduced on October 4 by the NS government, walking group participants are asked to provide proof of vaccination
in order to join a walking group. They can simply bring a copy of their COVID-19 vaccine receipt to their first walk to show the leader. They will only need to do this once.
